Counseling Services
If you would like to make an appointment to begin counseling with Dr. Heidi or if you have any questions related to her services, please contact her via phone or email to set up that first appointment.
Once you have your first appointment scheduled, (coming soon) please click the appropriate link below, print and complete these forms and bring them with you to your first appointment. If you are unable to complete these forms prior to your appointment, please arrive 15-20 minutes early to fill out these forms in the office. Please bring your insurance card or a copy (front and back) with you to your first appointment. Calling your insurance provider to determine your mental health coverage and copay amount prior to your first appointment is highly recommended.
